Job Description:

BASIC FUNCTION: Conduct comprehensive assessments of alleged victims of abuse, neglect, or exploitation (ANE), as well as cases of self-neglect (SN). Provide casework and follow-up services and interventions to mitigate on-going concerns regarding ANE and SN.

1. Receive and document reports of ANE and SN of older adults or adults with disabilities.
2. Ensure case consultation with APS Program Manager at the point of intake, after initial face-to-face, when determining substantiation status and developing care plan, at the time of case closure, and throughout the life of the case as determined by the APS Program Manager.
3. Conduct face-to-face assessments with alleged victims within identified timeframes.
4. Assure ongoing case management and follow-up services for vulnerable adults on designated caseload.
5. Assist with assuring a 24-hour response for crisis intervention for priority one reports of ANE or SN of vulnerable adults.
6. Complete appropriate referrals for follow-up services and interventions.
7. Develop and implement presentations on issues related to Adult Protective Services program.
8. Represent GACS in academic, community and professional organizations, as required.
9. Submit all reports, oral and written, in an accurate and timely manner as required by GACS and in accordance with contractual obligations.
10. Communicate problems and issues within the program to the Program Manager.
11. Work collaboratively with other agencies to facilitate services to clients and spread awareness of APS program.
12. Effectively manage a caseload of 20-25 cases (may be modified as needed).
13. Assist GACS administration in all investigations, as necessary.
14. Conduct other duties as assigned.

Job Requirements:

1. A Bachelor’s Degree in health, social sciences, social work, criminal justice or a related field.
2. Successful completion of APSCW Phase I and Phase II certification training and ACTIVE status on the Department’s registry.
3. Completion of twelve (12) hours of documented in-service training within a calendar year on topics of abuse of older adults and adults with disabilities, rights of older adults or adults with disabilities, self-neglect, or domestic violence victims.
4. Documented completion of eleven (11) hours of qualifying Recertification training every three years.
5. Excellent verbal and written communication skills.
6. Excellent computer skills.
7. Demonstrate sensitivity to the cultural and socioeconomic characteristics of persons served.
8. Ability to exercise discretion and independent judgment.
9. Ability to handle sensitive and confidential materials.
10. Ability to work flexible hours.
11. Valid Illinois driver’s license and proof of automobile insurance coverage.
