Job Description:

The Community Health Worker position responsibilities entail providing information regarding VNA services, collection of consents, performing immunization screening and traffic control as well as providing health education and guidance to interrupt ongoing disease transmission. Position works onsite at VNA health centers and/or at community sites in DuPage, Kane, Will, and Grundy counties.
1. Adapt or develop COVID-19 education and outreach, to conduct contact tracing support: connect cases and contacts with critical services and resources, such as meals, medicine, mobility support, immigration matters, work and income resources, mental health support, support for unsafe living conditions (e.g. domestic abuse), etc., during isolation and quarantine; and build vaccine confidence and assist in COVID-19 Vaccine registration.
2. Collaborate and coordinate across the assigned region with the public health system, including with LHDs, to ensure all communities receive needed assistance and to help reduce disparities in health outcomes.
3. Utilize IDPH-approved technology solutions, including Salesforce and engage in workforce development opportunities that are offered through IDPH.
4. Services funded pursuant to the grant should aim to reduce health disparities and health inequities where they exist.
5. Provide significant services and resources in your service area as well as hiring a community-based workforce that reflects the community demographic.
6. Sub-grantees will pay the PHNs hired under this award with a living wage and provide employee benefits, including health insurance.
7. Submit monthly fiscal (expenditure documentation forms) and programmatic status reports to IPHCA’s PHN Regional Lead for each region that you are responsible for, by close of business on the 10th of the month for the work done and expenses incurred in the previous month. Forms will be provided by IPHCA.
8. Ensure programmatic status reports effectively reflect monetary support distribution when Sub-grantee is receiving IDPH COVID-19 funding from multiple resources and assure that activity counts are not duplicated.

Job Requirements:

1. High school diploma or General Education Development (GED) equivalent
2. Contact Tracer Certification preferred, not required.
3. Experience conducting telephone-based or in-person interviews, data collection and data entry are preferred.
4. Experience with health education, community outreach, linkage to care, social services, or other public health services preferred.
5. Social work/case management experience preferred.
6. Ability to read and write in English required. Exceptional verbal and written communication skills.
7. Bilingual skills – fluency in Spanish (read, write and speak) preferred, not required.
8. Proficiency with use of computers (desktop, laptop, tablet).
9. Proficiency with Microsoft applications including Word, Excel and PowerPoint.
10. Proficiency with using the internet and telephone to include mobile devices.
11. Ability to adapt to changing environments and receive constructive feedback
12. Valid Driver’s license in the State of Illinois.
