Operates as a member of Community Services team. Provide outreach to Permanent Supportive Housing (PSH) candidates and daily support and monitoring to existing PSH residents. Provide initial intake and assessment, crisis intervention, and assistance to those seeking shelter. Provide assistance to Case Managers to support ongoing case management functions.


Provide outreach to potential PSH clients and referrals for services.
Coordinate the provision of center and meals to participants, including intake, and crisis intervention in emergency shelter.
Maintain safety and security of staff, participants, and volunteers by frisking, random locker searches, and monitoring safety.
Screen hotline calls. Direct them to appropriate staff or provide a referral.
Provide screening, assessment, crisis intervention, referral and linkage such as hospital, police, crisis line, etc.
Communicate DAYBREAK policies and regulations to participants through direct communication and consistent compliance with established procedures, including appropriate consequences.
Register participants and volunteers. Maintain statistical records and documents. Maintain paperwork as required.
Assist volunteers; provide meals, hospitality, and program assistance.
Provide assistance for residential clients, as needed.
Provide relocation assistance to clients, as needed.
Participate in staff and team meetings.
Work may be assigned in Shelter or Family Shelter programs.
Enter accurate documentation into the HMIS system.
Provide assistance for residential clients’ needs.
Schedule laundry, assist clients make phone calls, assign and check chores, passes, etc.
Maintain good relations with outside community professionals.
Complete daily filing of discharged residential files for Out of County, PADS clients and other residential clients as needed.
Assist Case Managers with other duties as needed to support client progress.
Assist with discharges of residential clients as needed and directed.
Perform other duties as assigned.

Job Requirements:

Must have High School Diploma or equivalent.
Must have one (1) – three (3) years experience in Human Service Field.
Must be at least twenty-one years old.
Demonstrated capacity to work with individuals/families in crisis situations.
Demonstrated capacity to work with persons from diverse social backgrounds.
Capacity to maintain flexible work hours as needed.
Ability to relate to clients in a warm, non-judgmental manner.
Willing to adhere to social teachings of the Catholic Church in performing work responsibilities.
Basic computer skills.
Understanding and good operational boundaries.
