Job Description:

Functions as a professional member of the Will County Community Health Center Clinical team. Candidate will perform a variety of functions to support quality health care services. Provide acute and chronic direct patient care services, while working with Primary Care Physicians, and other members of the health care team. Improve patient access and supports the quality initiative process. Part of the time will be spent coordinating and delivering primary health care services outside of the clinic on the mobile medical van. Ability to work flexible hours including evening and weekends.

Job Requirements:

Accredited Master's Degree in nursing. Two (2) years of experience as a nurse in clinical setting. Graduate from an accredited Nurse Practitioner Program. National certification as a Nurse Practitioner. Current Illinois CNP license. Current Illinois Registered Nurse License.


REQUIREMENTS: Verification of U.S. citizenship or employment eligibility. Must posses valid Illinois Driver License and minimum auto liability insurance.

SPECIAL REQUIREMENTS: One (1) year experience as a Family Nurse Practitioner in a clinical setting. Maintain current license and certification for CPR.
