Job Description:

Provide staff orientation, training, and development to assigned staff. Conduct staff performance evaluations as scheduled. Meet regularly with assigned staff.
•Develop, implement and update the individual training plan and delivery of services for each client in assigned caseload. Promote responsiveness to the needs & preferences of the client.
•Ensure that case records are complete and maintained in an orderly manner, ensuring that all required documentation is accurate and provided in a timely manner.
•Conduct formal staffings and other meetings with the Interdisciplinary Team to evaluate client progress and revise the individual training plan. Ensure formal assessment instruments are given to assigned clients as scheduled by agency policy. Monitor the implementation of behavior plans, suggesting changes as needed.

Job Requirements:

Serve as liaison to state operated facility staff for assigned case load, attend staffings and ensure coordination of day program and residential program plans.
•Ensure the timely and accurate preparation of reports requested by the Coordinator.
•Participate in Case Record Review / Program Quality Assessment Committee meetings, special team meetings, and all program meetings as directed.
•Facilitate the exit, discharge process and arrangements for follow-up and appropriate supportive services to individuals who terminate from the program.
•Additional duties as assigned.
