Job Description:

Coordinate implementation of assigned clients' Service Implementation Strategies, including completing all related documentation. Ensure appropriate training occurs leading to progress in meeting individual goals. Arrange for special staffings with the client, other staff, family and/or guardians, and others. Amend strategies, as indicated.
•Provide supervision and guidance to the assigned Lead Direct Support Professional and direct care staff, ensuring smooth operation of the homes in accordance with established policies and procedures.
•Interview, hire, train, evaluate, and discipline staff in accordance with personnel policies and procedures, under the direction of the Coordinator of Residential Services.
•Provide services and supports as identified in each resident's Service Implementation Strategy, clearly documenting services in client records at least monthly and more frequently as may be required.
•Maintain files and records as directed, insuring documentation is available validating operation in accordance with established policies. Insure state departmental (DMH, DPH, DPA) reporting is timely completed and that it documents clients' progress toward achieving established objectives. Perform periodic quality control audits for client records, as assigned.
•Complete and/or monitor completion of all annual assessments (SAMA, ICAP, SLOF, license reviews, physical, dental, etc.).
•Oversee budget planning process with clients, assuring that assets are used to meet financial obligations and that expenditures are appropriate and within the client's means.
•Maintain awareness of client's health and activity levels, assuring that each client promptly receives needed medical treatment from community providers as available resources allow.
•Maintain an active liaison with family members or significant others regarding the client's adjustment, progress, etc.

•Monitor staff who supervise clients to assure that clients develop and practice appropriate daily grooming and hygiene skills.
•Monitor staff who supervise client meal planning and preparation.
•Communicate on a regular basis with other program staff to assure consistency in operations.
•Utilize the agency's online work order system to notify the Buildings and Grounds Department of necessary repairs or other structural needs in homes.
•Provide counseling and crisis intervention to clients in need of such assistance and guidance.
•Assure the timely collection of monthly rent from each client in agency owned homes.
•Manage scheduling of professional consultants in providing services to individual clients.
•Oversee the timely submission of client service hours (attendance, CLSA billing, transportation billing, etc.) through the paperless reporting system.
•Operate agency or personal vehicle in accordance with established policies and procedures.
•Be familiar and act in accordance with established safety regulations, policies and procedures.
•Respond per established procedures to all emergency situations including medical, behavioral, fire, severe weather and other hazards.
•Maintain all required certifications, attend all assigned trainings, complete monthly online trainings.
•Maintain excellent on-time attendance.
•Perform other unspecified tasks and duties as assigned.
